The Immunizations tool helps you keep track of all your immunizations delivered by Pharmacists and public health services in Alberta. This information is updated frequently.
Immunizations are up-to-date as of the date and time indicated by “last updated.” Contact MHR Support at 1-844-401-4016 if your information is wrong or not up-to-date.
You can add past immunizations by clicking the “red button”
to add an immunization. You can make note of the date you had the immunization, when you need another dose (called the “re-immunization due date”), who gave the immunization (called the Administrator), and any other important notes.
By clicking on an immunization in the list, you have the ability to edit or delete it. Please note that this only applies to immunizations that you have added.
The “Set up Notifications” blue button
allows you to receive updates about your Immunization record or send notifications to someone you trust. If you want someone you trust to get notifications about your immunizations, share your record with them first so they can see your updates.

The blue "Print Immunizations" button
allows you to create and print a PDF report of your information.
Click on “Print Immunizations,” select your preferred date range (all days or anywhere between 7 days to 12 months), and save your selection.
You can also select what type of information you want to see printed on your report (allergies, appointment information, blood glucose, etc.)
When your selections have been made, click the blue “Export” button to produce the report. The PDF report will be created and you can save anywhere on your device.

My Personal Records users have access to test results ordered from all sites in Alberta, including labs obtained during inpatient and emergency visits.
Your test results will be available to you as soon as they are available, which is usually within 24 hours.
New users will see an 18-month retrospective view of any lab results slated for release. Existing users will not have labs added retroactively, but they will be able to see the expanded lab results moving forward.
