The Allergies tool allows you to keep track of your allergies. By clicking the “red button” in the bottom right, you can add an allergy.

You can add the type of allergic reaction (such as hives, rash, itching, tongue and throat swelling, or vomiting), date and time of the reaction, what to do, and other important notes.

Enter all required information, select from the drop-down list, and click “Save” when finished. 

You can view, edit, or delete an allergy listed by clicking on the allergy at any time.

The Appointments tool allows you to keep track of your past and upcoming appointments.  By clicking the “red button”  , you can note who the appointment is with, the reason for the appointment, date and time, and any other important details. Click “Save” when you are done.

You can view, edit, or delete an appointment by clicking on the appointment at any time.

The Family History tool lets you track information about your family’s health history. This is helpful in monitoring health conditions early or preventing you from developing a health condition that you may be at risk of getting.

Click the “red button” to add a family member, their relationship to you, their health condition, and other important notes.

Click on a family member in the list to edit or delete their health information. You can add more conditions for a family member when you choose them from the list, select “Edit,” and then “Add conditions.” Once completed, click “Save” and close.

The Immunizations tool helps you keep track of all your immunizations delivered by Pharmacists and public health services in Alberta. This information is updated frequently.

Immunizations are up-to-date as of the date and time indicated by “last updated.” Contact MHR Support at 1-844-401-4016 if your information is wrong or not up-to-date. 

You can add past immunizations by clicking the “red button” to add an immunization. You can make note of the date you had the immunization, when you need another dose (called the “re-immunization due date”), who gave the immunization (called the Administrator), and any other important notes. 

By clicking on an immunization in the list, you have the ability to edit or delete it. Please note that this only applies to immunizations that you have added.

The “Set up Notifications” blue button allows you to receive updates about your Immunization record or send notifications to someone you trust. If you want someone you trust to get notifications about your immunizations, share your record with them first so they can see your updates.

The blue "Print Immunizations" button allows you to create and print a PDF report of your information.

Click on “Print Immunizations,” select your preferred date range (all days or anywhere between 7 days to 12 months), and save your selection.

You can also select what type of information you want to see printed on your report (allergies, appointment information, blood glucose, etc.)

When your selections have been made, click the blue “Export” button to produce the report.  The PDF report will be created and you can save anywhere on your device.

Once you have made your selection, click "Save".

My Personal Records users have access to test results ordered from all sites in Alberta, including labs obtained during inpatient and emergency visits.

Your test results will be available to you as soon as they are available, which is usually within 24 hours.

New users will see an 18-month retrospective view of any lab results slated for release. Existing users will not have labs added retroactively, but they will be able to see the expanded lab results moving forward.

Alberta Immunizations List

The Medical Conditions tool allows you to keep track of your health conditions, such as asthma, diabetes, and high blood pressure. 

Click the “red button”  to add a medical condition, the date of diagnosis, treatments, and other important notes. As you begin typing the medical condition, common associated words will appear and you can select or self-enter your own.

You can edit or delete a medical condition by clicking on the drop-down list. If you no longer have a certain medical condition, click the “No longer have this medical condition” box. Enter the diagnosed date, time and any additional notes you want to include.

You will see two sections in the Medications tool.

When you start using MyHealth Records, the Alberta Netcare Prescription Medications section will show your prescriptions for the last 18 months. Newly dispensed prescriptions from most pharmacies show up every time you login.

Updates to this section will show your prescriptions up to 11:59 p.m. as of the date and time indicated. You cannot change or edit this information. Contact MHR Support at 1-800-315-6028 if your information does not look right.

Medication records from Alberta Netcare are updated every day.

Dispensed Prescription Medications

This section shows your medications which includes:

• Medication information
• Your refills

The name of any drug that has been discontinued will be struck through.

You can see more information by clicking the small "information icon" button . You can also click on a specific medication that appears in your record. This will open into a new screen so the information is easier to read.

The blue "Set up Notifications" button allows you to receive updates about your Medications or send notifications to someone you trust. If you want someone you trust to get notifications about your medications, share your record with them first so they can see your updates.

Other Medications

You can add other prescriptions, over-the-counter medications, and vitamin or herbal supplements.

You can add medications by clicking on the "red button" 

You can enter the following information:

• Medication name
• Date the prescription was filled
• The day you started your over-the-counter medication, such as vitamin or herbal supplement
• Strength (such as 500 mg)
• Dosage (such as 1 tablet or 2 drops)
• Frequency (such as once a day)
• Route (for example by mouth or inhaler)
• Date you discontinued taking the medication

You can click on any medication in the list to edit or delete it.

The blue "Set up Notifications" button allows you to receive updates about your medications or send notifications to someone you trust. If you want someone you trust to get notifications about your medications, share your record with them first so they can see your updates.

The blue "Print Medications" button allows you to create and print a PDF report of the information in MyHealth Records. You can select a specific date range and the type of information you would like to print.

The Procedures tool allows you to keep track of your health procedures, such as diagnostic imaging tests like x-rays or ultrasounds; lung and heart exams; surgeries, and other medical procedures.

You can add a procedure by clicking the "red button"  . You can note the following:

• Date and time
• Name of procedure
• Healthcare provider who performed the procedure
• The facility where you had the procedure
• The result of the procedure
• Where on your body you had the procedure
• Any other important notes

Click on a procedure in the list to edit or delete it.
